Forms for FPOs (13 documents)

Form 1: Acknowledgement of persons or organisations in relation to FPP:
This form should be used to obtain signatures for the FPP.

Form 2: Acknowledgement of persons or organisations with primary responsibility for management of forest practices:
This form should be completed where responsibilities are allocated under an FPP to any party that is not also the applicant, i.e. if the applicant is taking on all these responsibilities there is no need to have these forms completed.

Form 3: Acknowledgement of persons or organisations with specific responsibilities under FPP:
Variation of form 2. This form should be completed where responsibilities are allocated under a plan to any party that is not also the applicant, i.e. if the applicant is taking on all these responsibilities there is no need to have these forms completed.

Form 4: Acknowledgement of persons or organisations operating under FPP:
Completion of this form is optional. However, it is suggested that contractors be asked to complete this form as an acknowledgement that they are aware of the existence of the FPP, and the need to comply with its provisions.

s22 application for variation of certified FPP by FPO:
This form is to be used for any new variation initiated by the FPO. Remember to use the section 23 form for variations initiated by the applicant. The section 22 forms are only for use when you, as the FPO, determine that a variation is required (and generally where the applicant is not agreeable to initiating a variation under section 23).

s23 application for variation for certified FPP for FPP applicant:
This form is to be used for any new variation initiated by the Applicant.

Certificate of compliance form Word doc:
Use this form for submitting a certificate of compliance to the FPA.

Explanatory notes for drawing up quarry FPPs:
Quarry FPP proforma instructions (the cover page for quarries and the acknowledgement forms for quarries are the same as for all other FPPs).

FPP body template:
This form should be used for all FPPs. Minor headings can be deleted if not applicable to a particular plan, or modified to suit each organisations requirements.

The FPA has clarified the circumstances under which the FPA may assist an FPO with ecological assessments for conversion proposals in this policy document.

Form for applying for FPO Inspecting appointment:
Use this form for applying for FPO Inspecting if you have successfully completed the FPO Training Course.

Form for applying for FPO Inspecting appointment through RPL:
Use this form if you have not successfully completed the FPO Training Course and are using recognition of prior learning (RPL) in your application. Please note that this application process is designed for people with many years of experience who have not completed the FPO Training Course. It is not suitable for people who have participated in the FPO Training course but have not completed it to a satisfactory standard. Please contact the FPA about RPL before you fill in this form.
